$280 new patient evaluation, $240 follow up session
What is the length of a session?
50-60 minutes
Do you take insurance?
I am a out of network provider and do not take insurance. I can provide you with a superbill which you can submit to your insurance (not Medicare) for out of network reimbursement. It is your responsibility to understand your out of network benefits and pursue reimbursement for your session.
What kind of payment do you accept?
My booking platform uses Stripe and accepts all major credit cards and HSA cards. I also accept cash and check.
Do you have a cancellation fee?
If you cancel within 24 hours of your appointment time or are a no show you will be charged 50% of the cost. If you continue to have last minute cancellations you will be charged the full amount after a discussion is had. With that being said, I know things come up and people get sick. Please reach out to me via text or email if you are cancelling for an emergency situation or are sick and exceptions will be made. I prefer you do not come if you are sick.
My scheduling system is set to email you a reminder 2 days before your appointment and text you 24 hours and 2 hours before your appointment to avoid this fee.
Do I have to pay before I come?
I require a $100 deposit for new patient evaluations that will go towards your first session. All other sessions are paid for after services are rendered.
What to expect for a pelvic floor evaluation?
In addition to a regular orthopedic examination, as a pelvic floor specialist I can examine the pelvic floor muscles internally to check for strength, trigger points causing pain and coordination issues just like any other muscle we examine in the body. This is only done if medically necessary and with patient consent and comfort with the process. Doing this is typically extremely valuable for many of the common pelvic conditions I treat listed above but is my no means necessary.
Where are you located?
I am practicing at a private residence in Los Gatos, CA about a 1 minute drive from the intersection of Los Gatos Blvd and Los Gatos Almaden Blvd. After booking and paying the $100 initial evaluation deposit, you will receive the address and details for arriving.
